This table was a re-themed table made by Belgamko, Belgian Gaming Company, for Zonderik. It was based on the Seawitch table by Stern from 1980
 
Thanks to Luc for sending me the images used in the real machine.
 
I have included a picture of the real machine, since it is not in the IPD.

Could not find a good picture on internet, and recreating was a bit hard for me.
