Camelot from Bally.
 
It features:
rollover buttons (up top) to lite the A bumpers or B bumpers (they score more points when lit)
rollover buttons to raise and lower the center "PlayMore" post
rollover buttons to lower and raise the "gate" post.  Opening the gate allows a return to the shooter lane
The hit targets advance the saucer bonus on the left (when lit)
The "50 or 500" bonus on the right saucer rotates every time you hit a 1 point object (rubbers, unlit bumpers and rollovers).
An extra ball ("shoot again") can be earned by hitting the left lane when it is lit.

You use the F12 menu.
