I was having this issue too. I went in the B2STableSetting.xml and changed <FormToFront>1</FormToFront> to <FormToFront>0</FormToFront> and it fixed it for me.

OMG my heads going to explode. Nothing is working. The DMD only shows up for the tables that come pre installed. This is sick

Issue was self-inflicted. I selected full DMD and not slim dmd. I made the proper adjustments and abracadabra, the DMD showed up.
