With permission from 'R-DuB' I've updated his db2s backglass v1.1 for this table, following the directions from 'marauder' in the forum:
 
"The ROM lights are mapped incorrectly. You are using the manual of the machine as reference and the manual is incorrect. Extra Ball 1, 2 and 3 are 13, 14 and 15 respectively. Game Over is 32.".
