Settigs for high framerate G-sync play

I'm struggling to find working settings for my system. I have a 165hz G-sync monitor and I can get all my tabes to run 165fps easily for super smooth gameplay. The problem is that I get constant micro stutters and I can see the fps change constantly, all the way down to high 150's. If I cap the fps to for example 144, the fps fluctuates somewhere in the high 130's. No matter how much I drop the cap, the game aways runs lower. Enabling or disabling vsync doesn't help. Anyone with a gsync monitor and a constant unfluctuating fps?

Edit: to make things clear, the game runs at a high framerate, but even at 165hz, missing a single frame breaks the illusion as it doesn't feel smooth like locked 60fps for example.

Edit2:I'm playing with single portrait monitorin borderless window mide with a DMD window within the same screen. I have not been able to run in exclusive full screen mode (table crashes before starting).

For exclusive full screen mode

You have to go into windows registry and turn the D-draw off for each game (rom)

Open the registry and locate Computer\HK_LOCALUSER\Software\Freeware\Visual PinMame\<rom name

Gsync at this moment is broken for borderless window games.

Thanks. I disabled the G-sync and now it plays nice and smooth @165hz
