Based on the table by Bally from 1977
This is the FS version.
This version includes higher resolution images. If the playfield and plastics don't look good on your computer then just extract the images from the 4:3 table and import them to this table.

Line 138:
if Controller.Dip(0) <1 then Controller.Pause = true:msgBox "Please select your game options!" & vbNewLine & "You will only have to do this once!":vpmShowDips:Controller.Pause = false
Cancel out this line by putting a ' in front of it..
'if Controller.Dip(0) <1 then Controller.Pause = true:msgBox "Please select your game options!" & vbNewLine & "You will only have to do this once!":vpmShowDips:Controller.Pause = false
This will clear up the menu at startup. Not sure if it has any negative side effects.
The real fix would be to get Controller.Dip(0) to be equal to, or more than 1. I assume that once you enter your settings once, this is supposed to happen.
Had this problem too, since no solution was posted here I copied this from the forums. Thanks go to Rawd for the solution. BTW, you can still hit F6 in the game and change any of the dips so no adverse effects.
Thanks JP!
Thanks Shadowsclassic -> thanks Rawd.
Thanx! Nice table, reminds when I was young.
